Admission Process – New Students

The following items will then need to be submitted via email to lhawkins@archchicago.org in order to hold a spot for your student:
- Child’s birth certificate
- Baptismal Certificate (if applicable)
- Current Completed Child Health Examination – Click here to download this form
- Most recent report card (Grades 1-8 only)
- Records Release Form (Grades 1-8 only) – Click here to download this form
Once enrollment is complete, you will be required to create an account at FACTS Tuition Management.
Admission Process-Returning Student
Returning students can register for the 2024-2025 school year by clicking here
Age Requirements:
Three year old PreSchool – student must be 3 by September 1st
Four year old PreKindergarten – student must be 4 by September 1st
Five year old Kindergarten – student must be 5 by September 1st
State Health Requirements:
Physical– All students entering Pre-School, Kindergarten, and 6th grade are required to have a physical.
Vision– All students entering Kindergarten are required to have an Eye Exam.
Dental– All students entering Kindergarten, 2nd, and 6th Grade are required to have a Dental Exam.
